Teen Volunteer Application in PDF or MS Word format

(If you are prompted for a network password, click "Cancel".)

The application officially registers volunteers with the Library and the City of Oakland. It also provides for insurance and for identification of an emergency contact. The application helps to assess volunteer skills and to match applicants to available Library volunteer opportunities.

Volunteers under 18 years of age need a permission slip signed by their parent or guardian. This "Youth Volunteer Permission Slip" is part of the Teen Volunteer Application.

Adult volunteers are required to list two personal references on the application form.
